There wasn't a whole lot gained by sharing a common base class, and it makes
extending the manaserv Message{In,Out} classes with a debugging mode
unnecessarily complicated.

This required moving to C++0x, so it does that too, and fixes a few errors with
that.

Now the instance doesn't need to be passed into the MessageOut class
anymore. Expect a lot of cleanup in the next commit.

This merge involved major changes on both sides, and as such took
several weeks. Lots of things are expected to be broken now, however, we
now have a single code base to improve and extend, which can be compiled
to support either eAthena or tmwserv.
In the coming months, the plan is to work towards a client that supports
both eAthena and tmwserv, without needing to be recompiled.

This dates back to the old days of TMW, but the usage instructions of
GPLv2 don't mention this being necessary. Since it doesn't add anything,
avoid the branding in these sections.

WARNING!!! This, and all other previous builds have a linker error for
the Gnome libraries version 4.3.2 on my setup. It's assumed that this is
also the case for other users of this library as well. I'm currently
assuming that there's a bug in the compiler itself, and will look into
reporting this, but in the mean time, it doesn't build for these users,
unfortunately. Sorry about this.

Removed unused Packet class. Added stub for handling GPMSG_BEING_ENTER.

the server type as a parameter. The MessageOut gained a convenience constructor
(same as was added server side). The game states during login sequence have
been renamed and redone in order to ensure no communication is attempted to
unconnected servers. This allowed the removal of the outgoing message queue.
Connecting to the account server has been moved before the login/register phase
(dialogs will still need to be updated). Quite a few things are expected to be
broken since I'm rather tired at the moment. I've left many TODO entries in the
code.
