Shows outlines of visible widgets as well as highlighting modal focus,
modal mouse focus and regular focus.
Can currently only be toggled in the Debug window, so needs source code
modification to be used during login sequence for now.

* Use default member initializers
* Use range-based loops
* Don't use 'else' after 'return'
* Removed some unused includes
* Construct empty strings with std::string() instead of ""
* Clear strings with .clear() instead of assigning ""
* Check whether strings are empty with .empty() instead of comparing to ""
* Removed redundant initializations

Instead, support for dynamically adjusting layout was added to the
Container class.
Various other places were also adapted to use the new layout support in
Container.

The class DebugSwitches is a derived class from Container, 'which
automatically deletes its child widgets, so there is no need for
such fragile cleanup code.'

Their name is a bit more clear with DEBUG prefix rather than MAP
prefix. They're already scoped in the Map class anyway.
MAP_NORMAL was changed to DEBUG_NONE to represent no debug flags.

* The f-key is no longer used. That means new players are less likely
to get into trouble by accidentally activating the debug mode.
* The debug mode can now be activated in a new tab in the debug window.
* The main advantage of using a gui is its extensibility. At the moment
the debug mode does show too much information at once. In a follow-up
patch the user should get more choices.

Much code was moved from main() to the new Client::exec(). This new
event loop now integrates with the Game class, so that the tick counter
and framerate limiter apply universally.
The Client class is also responsible for some things that used to be
global variables.

There was little point in keeping the Engine class separate. It wasn't
an engine at all, but only kept track of the currently active map, a job
more suitable for the Game class anyway.

Moved remaining widgets into widgets folder, standardized include order, moved TextRenderer out.

This has confused many players, as we at times have believed them
to be the player coordinates and not those of the cursor.
(A possible extension might be to put the player coordinates as
well in the debug window or elsewhere, for instance the minimap.)

In an attempt to make the GUI code a little more structured, basic
widgets are now put in gui/widgets. Many includes were also cleaned up.

the palette colors. While technically, this can be accomplished through
other means, it's rather clumsy overall, and is prone to introducing in
programmer errors.
This commit finally catches every case where text is used, and applies
the text color from the color dialog to each of them appropriately.

include anything not needed by that specific widget or window. This
appears to have cleaned up system performance a bit on my current setup,
where it went from idling on 45% in game with opengl down to 30% now.
Also moved iptostring to the tostring header, as importing all of
network.h is a little overkill to use that function, and it goes along
with the basic functions that are in that header file anyways. TODO:
find out a way to get rid of warnings when a class doesn't use this
function.
